Home Health Physical Therapist Position

LHC Group is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes.
About the RoleThe Physical Therapist will be responsible for assessing and evaluating patient care needs, developing treatment plans, and performing interventions to improve patient well-being.
- Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
- Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification required.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
