Role ResponsibilitiesThe Clinical Care Nurse will deliver essential primary and urgent care services to adult patients, in alignment with their qualifications, at one of PHMC's Medical Respite Programs.
This role involves:
- Triage and Assessment: Conducting evaluations of acute health issues and coordinating care with community healthcare providers.
- Collaboration: Working closely with the Mary Howard Health Center and other PHMC Health Center personnel to ensure comprehensive patient care.
- Patient Management: Administering medication management in accordance with established medical respite standards.
- Infection Control: Adhering to ambulatory care guidelines to maintain a safe environment.
- Client Support: Connecting clients with necessary primary care, behavioral health, and social services.
- Education and Advocacy: Providing health education and advocacy services to patients and their families.
- Documentation: Maintaining accurate records and adhering to compliance standards.
The ideal candidate will possess:
- A Bachelor's degree in Nursing from an accredited institution.
- A minimum of three years of nursing experience, preferably in primary or community health.
- Strong verbal and written communication skills.
- Proficiency in Microsoft Office Suite and familiarity with Electronic Medical Records.
- The ability to prioritize tasks effectively in a dynamic environment.
