About the Role

Pine Park Health is seeking a dedicated and compassionate Nurse Practitioner to join our team in Sonoma County, California. As a founding member of our primary care team, you will be responsible for delivering high-quality care to older adults living in Senior Living Communities.

Key Responsibilities
  • Provide onsite primary care services to older adults, including preventative care, chronic condition management, and laboratory tests.
  • Develop and execute detailed person-centered care plans for all primary care patients.
  • Collaborate with our Medical Director to identify and develop policies addressing issues affecting day-to-day practice, culture, or clinical guidelines.
  • Work with Primary Care Coordinators to provide monthly Chronic Care Management visits and care, reviewing and signing off on CCM documentation.
  • Support diagnostic needs for each patient in communities with Traveling Clinical Assistants.
  • Lead regular huddles and provide leadership to the clinical team.
  • Identify opportunities to work with senior living management and care teams to better implement Pine Park care plans and improve patient outcomes.
Requirements
  • Board-Eligible or Board-Certified Physicians in Internal Medicine or Family Medicine, or Nurse Practitioner or Physician Assistant, preferably with geriatrics experience.
  • Comfortable with a fast-paced and changing environment.
  • Willingness to provide onsite clinical care across the market.
  • Desire to work with complex care patients and support the building of a holistic care model.
  • Strong orientation towards collaboration and working closely with a multidisciplinary team.
  • Strong written and verbal communication skills.
  • Ability to work with tech-enabled care delivery tools to improve patient care and team function.
What We Offer
  • A competitive salary range of $140,000 to $180,000, inclusive of base, annual performance bonus, sign-on bonus, plus equity and benefits.
