About the Role

We are seeking a skilled Physical Therapist to join our team at Adoration Home Health and Hospice. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Responsibilities
  • Develop and implement individualized physical therapy plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Provide direct patient care, including exercises, stretches, and other interventions to promote mobility and function.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and caregivers to educate them on treatment plans and progress.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in the state of operation.
  • 2 years of experience in adult physical therapy, with a preference for home health experience.
  • Commitment to clinical and documentation excellence.
