Job Summary:
Vaya Health is seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a critical role in providing proactive intervention and coordination of care to eligible Vaya Health members and recipients. Your primary responsibility will be to ensure that these individuals receive appropriate assessment and services, alleviating inappropriate levels of care or care gaps through assessment, multidisciplinary team care planning, linkage, and coordination of services needed by the member across various health domains.
Key Responsibilities:
- Conduct comprehensive bio-psycho-social assessments addressing social determinants of health, mental health history and needs, physical health history and needs, activities of daily living, access to resources, and other areas to ensure a whole-person approach to care.
- Develop and implement person-centered care plans that prioritize goals and services to address mental health, substance use, medical, and social needs.
- Collaborate with members, care teams, and other stakeholders to ensure seamless care coordination and transition planning.
- Provide crisis intervention, coordination, and care management as needed while working with members in the community.
- Support transitional care management responsibilities for members transitioning between levels of care and coordinate diversion efforts for members at risk of requiring care in an institutional setting.
- Consult with care management licensed professionals, care management supervisors, and other colleagues as needed to support effective and appropriate member care.
Requirements:
- Bachelor's degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
- Two years of experience working directly with individuals with behavioral health conditions, or two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience.
- Ability to express ideas clearly and concisely, communicate effectively, and represent Vaya in a professional manner.
- Strong problem-solving, negotiation, arbitration, and conflict resolution skills, with a high level of diplomacy and discretion.
- Ability to complete and maintain all trainings and proficiencies required by Vaya, including but not limited to, BH I/DD Tailored Plan eligibility and services, whole-person health and unmet resource needs, community integration, and components of Health Home Care Management.
Physical Requirements:
- Close visual acuity to perform activities such as preparation and analysis of documents, viewing a computer terminal, and extensive reading.
- Physical activity in this position includes crouching, reaching, walking, talking, hearing, and repetitive motion of hands, wrists, and fingers.
- Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
Residency Requirement:
This position is required to reside in North Carolina or within 40 miles of the North Carolina border.
Salary:
Depending on qualifications and experience of candidate. This position is non-exempt and is eligible for overtime compensation.
