Job Summary

As a Home Health Physical Therapist with Springfield Clinic, you will have the opportunity to make a positive impact on patients' lives while enjoying flexibility and independence. This rewarding role combines personalized patient care with the freedom to manage your own schedule.

The Home Health Physical Therapist evaluates, plans, and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.

Key Responsibilities
  • Provides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelines
  • Establishes an in-home treatment plan in coordination with the physician and other care team members
  • Utilizes various evaluation and treatment techniques based on both physical and psychosocial levels of patient function
  • Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines
  • Responsible for planning, evaluation, and treatment implementation of an assigned caseload
  • Provides quality, personalized and evidence-based care and proven interventions
  • Supervises therapy assistants and other home health staff as appropriate
  • Motivates patients during treatment in order to help them function optimally
  • Reduces the need for medications and provides alternatives to surgery
  • Evaluates effects and monitor and communicate progress
  • Consults with physicians regarding change in treatment
  • Writes reports to physicians regarding patient's progress
  • Instructs patients and family/significant others in home programs and activities of daily living
Requirements
  • Successful completion of a Physical Therapy program approved by CAPTE, Commission on Accreditation in Physical Therapy Education
  • One year of hospital or home care experience is preferred
  • Bachelor's degree or higher in Physical Therapy
Preferred Qualifications
  • Currently licensed by the Illinois State Board of Physical Therapy
  • Current AHA CPR certification is required
  • Reliable means of transportation, valid IL driver's license and vehicle liability insurance required
Working Environment

This job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car

Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions

Possible exposure to blood, bodily fluids and infectious diseases

This position is employed to provide services for White Star, a not-for-profit affiliate of Springfield Clinic
