Job Summary

Key Responsibilities
  • Generate and electronically file patient-related information for medical records, adhering to company policy and regulatory requirements.
  • Prepare mailings to physicians, ensuring timely receipt and return of medical orders.
  • Run Missing Plans of Care and Orders reports, working to obtain missing information and maintain accurate patient records.
  • Request medical records from doctors' offices, hospitals, facilities, and family members, as needed.
  • Print patient care plans, medication order sheets, and demographic information for patients being transferred to other facilities.
  • Support and attend weekly interdisciplinary team meetings, updating the electronic medical record with team documentation.
  • Prepare all documentation for IDT members, collecting and entering information into the EMR.
  • Prepare necessary paperwork for re-certifying patients for ongoing care, adhering to regulatory guidelines.
  • Review documentation for quality and accuracy, maintaining IDT meeting attendance records in Homeworks.
  • Conduct regulatory audits of patient charts, reviewing inactive patient medical records and closing records within 30 days of discharge.
  • Communicate with pharmacies, mailing/faxing prescriptions written during IDT, and respond to record requests from outside sources and the billing department.
  • Identify patient needs and regulatory requirements associated with medical insurance, printing MTM assessment forms for review by Clinical Managers.
  • Provide administrative support services, preparing daily reports for the Clinical Manager and gathering data for administrative and statistical reports.
  • Respond to phone calls, emails, and mailings in a timely manner, providing coverage for other teams as needed.
Requirements
  • High school diploma, GED, or equivalent required.
  • 1 year's customer service or administrative experience; additional education beyond high school may be substituted for experience.
  • Knowledge of medical terminology preferred.
  • General knowledge of office practices, procedures, and equipment; business English, spelling, punctuation, grammar, and basic arithmetic calculations.
  • Ability to use a variety of office software applications, such as word processing, spreadsheets, presentation packages, and database applications.
  • Strong communication and organizational skills, with attention to detail and ability to prioritize tasks.

Pay Range: $26.03. Final salary offer will be based on the candidate's qualifications, education, experience, and alignment with our organizational needs.
