AccentCare is seeking a skilled RN Case Manager to join our team in Alameda County. As a medical professional, you will have the opportunity to reimagine care and provide world-class patient care in a home health setting.
Key Responsibilities:
- Provide comprehensive care to patients in their homes, including assessments, care planning, and coordination of services.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
Requirements:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
- One (1) year experience as a RN.
- Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Ability to travel to all business locations.
