Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address patients' unique requirements
  • Provide emotional support and counseling to patients and their loved ones
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care
Requirements:
  • Masters degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program
  • Current state licensure as a Social Worker
  • Minimum 2 years of experience in clinical social work, preferably in healthcare or hospice settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
