Key Responsibilities- Comprehensive Assessment: Conduct initial and ongoing assessments of patients and their families to identify hospice care needs, including a thorough physical examination and medical history review.
- Nursing Care Delivery: Provide professional nursing care by applying all aspects of the nursing process effectively.
- Patient Evaluation: Regularly assess and evaluate the patient's condition by:
- Creating and implementing a tailored plan of care
- Continuously reassessing the needs of the patient and their family/caregivers
- Adjusting the care plan as required based on patient progress
- Care Plan Initiation: Launch the care plan and modify it as necessary to align with the evolving status and requirements of the patient.
- Nursing Diagnosis: Utilize health assessment data to establish nursing diagnoses.
- Goal-Oriented Care Planning: Develop a care plan that sets achievable goals based on nursing diagnoses, incorporating palliative care strategies and involving both the patient and family in the planning process.
- Preventive and Rehabilitative Care: Initiate appropriate nursing procedures aimed at prevention and rehabilitation, administering medications and treatments as outlined in the physician's care plan.
- Patient and Family Counseling: Provide guidance to patients and their families to address nursing and related needs.
- Health Education: Deliver healthcare instructions to patients as deemed appropriate based on assessments and care plans.
- Support with Daily Activities: Assist patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Case Management Role: Act as a Case Manager as designated by the Clinical Supervisor, taking responsibility for coordinating care for an assigned patient caseload.
