Job Summary

The Personalized Health Partners (PHP) Nurse Practitioner is responsible for delivering high-quality primary care in a personalized, patient-centered model. This role involves diagnosing, treating, and managing acute and chronic illnesses through wellness visits, early detection, and timely treatment.

Key Responsibilities
  • Assess and diagnose chronic and acute conditions
  • Provide health education and develop treatment plans
  • Order and interpret lab and diagnostic testing
  • Maintain accurate and compliant documentation
  • Collaborate with interdisciplinary teams and local operations leadership
  • Support strategic initiatives in value-based care
  • Provide exceptional customer experience and foster high customer satisfaction
  • Practice ethically and in accordance with professional standards and regulations
Requirements
  • Master's degree in Nursing or Doctor of Nursing Practice
  • National certification as an Adult Nurse Practitioner
  • Active, unrestricted state license
  • Active DEA license
  • US work authorization
  • 2+ years of clinical experience as a Nurse or Nurse Practitioner
  • Ability to work in a flexible environment and schedule
Preferred Qualifications
  • Training or experience in geriatrics
  • 3+ years of clinical experience as a Nurse Practitioner
  • Experience with coding, ICD-10, and HCC
  • Experience working in value-based care models
