Palliative Care Coordinator

We are seeking an experienced Palliative Care Coordinator to join our team at Blue Monarch Hospice LLC. As a key member of our interdisciplinary team, you will play a critical role in delivering exceptional patient care and ensuring the highest quality of life for our patients.
Key Responsibilities
- Conduct thorough assessments of patients and families to determine hospice needs.
- Develop and implement comprehensive care plans that address patients' physical, emotional, and spiritual needs.
- Collaborate with the interdisciplinary team to provide seamless care coordination.
- Provide education and support to patients and their caregivers on symptom management, end-of-life care, and other relevant topics.
Communication and Collaboration
- Maintain accurate and detailed clinical records, including pain and symptom management updates.
- Communicate effectively with healthcare professionals, patients, and families to ensure informed decision-making.
- Participate in interdisciplinary group meetings to discuss patient care plans and address concerns.
