Job Summary

We are seeking a highly skilled and detail-oriented Transfer Center Coordinator to join our team at Confluence Health. The successful candidate will be responsible for facilitating patient admissions, transfers from outlying facilities, and coordinating specialty consults.

The Transfer Center Coordinator plays a critical role in ensuring the safe and efficient transfer of patients from other facilities to Confluence Health. This involves managing communication, information, and processes to facilitate patient transfers, admissions, and/or specialty consults using evidence-based criteria, guidelines, and regulations.

Responsibilities

  • Evaluates and coordinates telephone calls from outlying facilities regarding patient admissions, transfers, and specialty consults in an efficient and timely manner.
  • Exhibits excellent communication and customer service skills with healthcare teams to facilitate patient care.
  • Operates closely with facility staff to maintain up-to-date knowledge on bed availability and capacity.
  • Participates in process improvement initiatives to optimize patient flow and implement efficient transfer processes.
  • Conducts recorded calls for quality assurance purposes, evaluates, and marks calls for review.
  • Able to efficiently perform multiple tasks to facilitate coordinated transfers and admissions.
  • Ensures accurate unit assignment of patients based on needed level of care.
  • Performs community outreach and marketing to improve communication, processes, and increase transfer volume.
  • Communicates effectively with the Director of Transfer Center, Medical Liaison, Administrator on Call, House Supervisor, and others.
  • Collects and maintains data to monitor quality, identifies factors affecting quality, and assists in creating process improvements.
  • Demonstrates adherence to Confluence Health's Code of Conduct in all job performance aspects.
  • Required to maintain active Basic Life Support (CPR) certification.

Requirements

  • High School Diploma or GED.
  • Completion of Department-approved paramedic training course.
  • Paramedic licensure.
  • Required continuing education, including knowledge and skills competency administered by the Physician Medical Program Director.
  • Current certification and maintenance of BLS.

Estimated Salary Range: $65,000 - $80,000 per year, depending on experience.
