**Key Accountabilities:

  • Providing direct casework and counseling services to patients and families
  • Assessing and addressing psychosocial problems in patients and families
  • Developing and implementing treatment plans tailored to individual needs
  • Collaborating with interdisciplinary teams to ensure comprehensive patient care


**Requirements & Qualifications:**

  • Masters or Bachelors degree in Social Work from an accredited program
  • Licensed by the state of Kentucky as a Licensed Social Worker (LSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW)
  • Minimum 2 years experience in social service or healthcare setting
  • Knowledge of death, dying, and bereavement processes
  • Basic understanding of psychosocial and legal factors affecting terminally ill patients and their families
