Ob Coordinator

Sea Mar Community Health Centers is seeking a highly skilled and compassionate OB Coordinator to join our team. As an OB Coordinator, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our patients.
Job Summary:The OB Coordinator will be responsible for coordinating the care of patients throughout their pregnancy and postpartum journey. This includes conducting prenatal intakes, documenting patient information, and providing education and support to patients and their families.
Key Responsibilities:- Conduct prenatal intakes and document patient information in the Electronic Health Record (EHR)
- Provide education and support to patients and their families on prenatal care, childbirth, and postpartum care
- Coordinate care with other healthcare providers and community resources to ensure comprehensive care
- Participate in daily huddles and identify patient needs according to protocols
- Provide point of care services and education to patients
- Work closely with the patient and clinical team members to ensure timely delivery of care
- Enhance quality and patient-centered care at Sea Mar Community Health Centers
- High school diploma and continued education in areas of health or social services
- At least one year of experience in a health and human services setting
- Knowledge of medical terminology and/or behavioral health topics
- Strong decision-making and prioritization skills
- Ability to work respectfully and professionally with diverse populations
- Knowledge of the Patient-Centered Medical Home Model and motivational interviewing skills a plus
