Job Summary

We are seeking a highly motivated and organized Care Coordinator Specialist to join our team at Go Kids Inc. The successful candidate will be responsible for managing required activities for integration and navigation, including outreach, engagement, screening, care coordination/case management, participation in multi-disciplinary review teams, reflective supervision, and documentation/data entry and evaluation.

The Care Coordinator Specialist will work closely with families, county agencies, and community partners to develop integrated and coordinated family success plans. Key responsibilities include connecting with community-wide service providers, coordinating with resource and referral entities, working with internal staff and community partners to streamline effective referrals, meeting with Family Services Teams to support enhanced referrals, providing information about community supports and services, using relationship-based strategies to develop an understanding of strengths, current services, and goals, and engaging in professional development and capacity building efforts.

In addition to these duties, the Care Coordinator Specialist will maintain client files, engage in evaluation, monitoring, and continuous quality improvement activities, attend required trainings, meetings, and learning community events, and demonstrate exceptional communication skills to present facts and recommendations effectively in oral and written form.

Requirements and Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, Child Development, or a related field
  • Two to three years' experience working with children/families; or equivalent combination of education and experience
  • Endorsement in Infant Family Early Childhood Mental Health (IFECMH) or working toward endorsement and/or Santa Clara County's Family Development Course units a plus

Salary and Benefits

The salary for this position is $66,250 to $70,625 per year, depending on experience. Additionally, the successful candidate will receive comprehensive benefits, including health insurance, dental insurance, vision insurance, employee assistance program, tuition reimbursement program, student loan repayment program, life insurance, paid time off, and retirement plan.
