Role Summary:
We are seeking a dedicated Rehabilitation Specialist to become a vital member of our clinical team. This role will be based at one of our various facilities, where we emphasize a holistic approach to healthcare.

Our commitment to delivering exceptional care for individuals with disabilities is central to our mission. As a Rehabilitation Specialist, you will work closely with healthcare providers, families, and interdisciplinary teams to assess functional capabilities and needs, thereby creating personalized and impactful therapy plans for those we support.

Key Responsibilities:
1. Collaborate with healthcare professionals, families, and interdisciplinary teams to assess functional strengths and needs.
2. Develop individualized, meaningful, and effective therapy plans for the individuals we serve.

Qualifications:
- Degree in Rehabilitation Therapy from an accredited institution.
- Active and valid licensure in New York State.
- While experience is beneficial, we welcome applications from recent graduates.
- Ability to work collaboratively within a team to provide comprehensive care.
- Proficiency in utilizing electronic medical records (EMR) systems.

Compensation:
Salary will be determined based on experience.
