Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Mon Health. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and managing health problems in accordance with state and federal regulations.

Key Responsibilities
  • Assess health status, diagnose, and develop plans of care and treatment for patients
  • Implement treatment plans and follow up with evaluations of patient status
  • Use clinical judgments to conduct health assessments, make diagnoses, and prescribe pharmacological and non-pharmacological treatments
  • Collaborate with other healthcare providers to identify changing patient needs and provide comprehensive care
Requirements
  • Master's Degree or Post-Master's Certificate in a relevant Nurse Practitioner Program
  • Certification by either ANCC or AANP Board, including completion of all requirements for Continuing Medical Education (CME) and re-certification
  • Previous primary care experience in a clinical setting is strongly preferred
  • Current license as a Nurse Practitioner from the West Virginia Board of Registered Nurses and current RN license
  • Prescriptive authority and credentialing according to the Grafton City Hospital Medical Staff By-Laws
Competencies
  • Collaboration: Interact with various members of the healthcare team, collaborate with other providers to identify changing patient needs, and identify the teaching/learning needs of patients
  • Patient Care: Develop and implement care plans based on findings and examinations, prepare patient summaries, collect specimens, and carry out the analysis of cultures and tests
  • Knowledge and Skills: Know what clinical procedures need done and how to do them, provide counseling and instruction regarding common patient problems, limitations, and abilities, and identify normal and abnormal findings in history, physical examination, and commonly performed laboratory studies
  • Technology: Have basic computer skills and demonstrate proficiency in documenting and looking up clinical information in electronic records
  • Safety: Protect patients, visitors, and employees by adhering to all policies and protocols, and identifying patients using two identifiers (i.e., name and date of birth) prior to providing any treatment or procedure
Physical Requirements and Environmental Conditions

This role requires frequent lifting and/or carrying objects weighing up to 50 lbs. with a maximum lifting of 100 lbs.
