Position: MDS Coordinator (RN License Required)

Employment Type: Full Time, Day Shift

Role Overview:

  • Ensure adherence to a comprehensive schedule for all MDS submissions.
  • Facilitate the collaborative completion of MDS documentation across various disciplines.
  • Assess MDS data for precision and engage with relevant personnel to guarantee accuracy.
  • Finalize MDS entries and ensure proper input into the electronic system.
  • Confirm timely transmission and approval of assessments by the State.
  • Address any rejected records and prepare them for resubmission.
  • Complete Care Area Assessments (CAAs) in accordance with state regulations.
  • Develop Resident Care Plans (RCP) in collaboration with the interdisciplinary care team, tailored to individual resident needs.
  • Organize and lead regular RCP meetings to ensure compliance and communication.
  • Disseminate RCP information to relevant staff and alert supervisors of any compliance issues.
  • Collaborate with all departments to ensure understanding and adherence to the RCP.
  • Perform additional responsibilities as assigned by the Director of Nursing or Administrator.

Qualifications:

  • Current RN license from an accredited nursing program and valid North Carolina Board of Nursing license.
  • Ability to comprehend and follow facility policies and adhere to applicable regulations.
  • Experience with MDS-RAP and Care Planning processes is essential.
  • Preferred familiarity with RUG-IV and MDS 3.0.
