**Locum Nurse Practitioner Opportunity**

We are seeking two (2) home health nurse practitioners (NPs) to join our team at Wellhart. As a locum NP, you will provide in-home assessments, including full health assessments, blood pressure checks, and medication overview. You will also provide referrals for specialty care and may be required to perform EKGs. Charting must be completed the same day. To ensure continuity of care for patients, we require a commitment to working 4-5 days/week, Mon-Fri or Mon-Thurs, for 16 weeks.
Key Responsibilities:
- Provide in-home assessments, including full health assessments and medication overview
- Provide referrals for specialty care
- Perform EKGs (equipment provided)
- Charting must be completed the same day
Requirements:
- Comfortable providing care to patients 65+
- Willing to commit to working 4-5 days/week, Mon-Fri or Mon-Thurs, for 16 weeks
