We are seeking a skilled Physical Therapist to join our team at Pathwell Health. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.
Key Responsibilities- Develop and implement individualized rehabilitation plans to improve mobility, strength, and function in patients.
- Conduct thorough assessments and evaluations to identify patient needs and goals.
- Provide direct patient care, including exercises, stretches, and other therapeutic interventions.
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care and support.
- Document patient progress and outcomes to inform treatment plans and improve patient care.
- Graduation from an accredited Physical Therapy program.
- Current licensure as a Physical Therapist in the state of Connecticut.
- Experience working in home health or a related setting preferred.
- Strong clinical assessment, organizational, and communication skills.
- Ability to work independently and as part of a team.
The Physical Therapist will work in a home health setting, visiting patients in their homes to provide care and support. The ideal candidate will be able to work in a fast-paced environment and adapt to changing patient needs.
