We are seeking a highly motivated and organized Nurse Case Manager to join our team. As a Home Hemodialysis Specialist, you will be responsible for providing comprehensive patient care in an independent setting. Your primary focus will be on empowering patients to take control of their health by teaching them to administer their own dialysis and promoting independence.
Key Responsibilities- Provide comprehensive patient care, including assessment, diagnosis, and implementation of care plans.
- Train patients to administer their own dialysis, promoting independence and self-management.
- Develop and maintain relationships with patients, support systems, and care partners.
- Manage patient cases, ensuring timely and effective care.
- Collaborate with healthcare teams to ensure seamless care transitions.
- Current driver's license and reliable transportation.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing.
- Basic computer skills and proficiency in MS Word and Outlook.
- Previous experience in chronic hemodialysis, acute dialysis, peritoneal dialysis, or home hemodialysis nursing.
- Training/Teaching/Healthcare Education background.
- Previous case management experience.
- Familiarity with telehealth technologies.
