Job Summary

We are seeking a skilled LPN/LVN to join our Home Therapy team as a Home Dialysis Therapist. As a key member of our team, you will provide high-quality patient care and support to patients undergoing dialysis treatment in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care to assigned patients, including administering medication and performing dialysis treatments.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Conduct home visits to assess patient needs and provide education on dialysis treatment.
  • Monitor patient progress and report any concerns to the healthcare team.
  • Ensure accurate documentation of patient information and treatment outcomes.
Requirements
  • Graduate of an accredited School of Practical Nursing (LPN, LVN).
  • Current appropriate state licensure.
  • One year medical-surgical nursing experience preferred.
  • Hemodialysis experience preferred but not required.
  • Successfully complete a training course in the theory and practice of hemodialysis.
  • CPR certification.
