Franciscan Physician Network is actively seeking a Nurse Practitioner specializing in Palliative Care for both inpatient and outpatient responsibilities. This role is essential to address the increasing need for Hospice and Palliative Care services.
Candidate Profile:
The ideal candidate will possess prior experience in Palliative or Home Health Care, demonstrating a commitment to clinical excellence and patient satisfaction. Exceptional interpersonal skills are crucial, along with a strong desire to foster relationships within the community and collaborate effectively with the existing medical team.
Role Expectations:
- Standard clinic hours: Monday - Friday, 8:00 a.m. - 4:30 p.m.
- Flexible full-time scheduling options available
- Average outpatient patient load: 12 patients weekly
- Average inpatient patient load: 6 patients daily
- Conduct inpatient follow-ups and new consultations
- Facilitate care planning discussions with patients' families
- Telehealth services are available
- Certification as a Hospice and Palliative Nurse (ACHPN) is preferred, with eligibility for certification required within two years

Qualifications:
- Master's Degree in Nursing - Required
