Job Title: Licensed Clinical Social Worker

At Principle Choice Home Healthcare, we are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team. As a key member of our interdisciplinary team, you will provide social work services to patients in their place of residence, working closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care.

Job Summary:

The Licensed Clinical Social Worker will be responsible for assessing patients' social and emotional needs, developing and implementing plans to address these needs, and providing support and guidance to patients and their families. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Conduct comprehensive assessments of patients' social and emotional needs
  • Develop and implement plans to address patients' social and emotional needs
  • Provide support and guidance to patients and their families
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care
  • Document all patient interactions and services provided
Requirements:
  • Master's or doctoral degree in social work from a Council on Social Work Education-accredited program
  • Current, unrestricted state license to practice as a social worker
  • Basic Cardiac Life Support (BCLS) certification
  • 1-2 years of experience in home health or a related field
  • Active Oklahoma driver's license and vehicle insurance

The ideal candidate will be able to work independently and as part of a team, and will possess excellent communication and interpersonal skills.
