Medical Specialist 2 Position

Key Responsibilities:- Actively participate in diagnosing and treating patients within your specialized field.
- Advise and confer with members of other disciplines to ensure patients receive comprehensive care.
- Develop and revise medical policy and procedures to ensure quality care.
- Review lab work, specialty consults, hospital discharges, and medical visit summaries to assist the treatment team.
- Discuss care with hospital or rehabilitation facility providers to ensure continuity of care.
- Review and approve discharge summaries prior to accepting individuals back into the group home residence.
- Review medical information for new admissions to ensure appropriate care can be provided.
- Communicate with team members regarding medical issues and participate in team meetings as needed.
- Communicate with family members and advocates to explain medical or treatment issues.
- Provide short-term urgently needed prescription renewals when a community medical provider is unavailable.
- Function as a member of an interdisciplinary treatment team, attending meetings and serving on committees to improve individual services and outcomes.
- Provide after-hours and weekend on-call medical telephone consult on a rotating basis.
- Provide coverage of primary medical care for people living in a Comprehensive Adult Transitional Home if required.
- Provide medical expertise to Family Care Program and Community Mental Health Nurses as needed.
- Assist with physician coverage as needed in sister regions.
