About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our Home Health team in Petersburg, VA. As a part of LHC Group, we provide high-quality care that supports our patients when and where they need it.

Job Summary

The Advanced Care Nurse Specialist will be responsible for providing quality patient care and performing technical skilled care in the patient's home. The ideal candidate will have strong communication skills, a commitment to delivering exceptional patient outcomes, and the ability to work effectively within a dynamic healthcare environment.

Key Responsibilities
  • Provide quality patient care and perform technical skilled care in the patient's home.
  • Instruct the patient's family in the care of the patient and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements

To be considered for this role, you must have:

  • A current LPN licensure in the state of practice.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification.
