You are a compassionate and committed professional holding an Associate's or Bachelor's degree in nursing, licensed as a Licensed Practical Nurse in the State of Ohio. A minimum of one year of relevant experience in psychiatric or community mental health services is preferred but not mandatory.
In this position, you will frequently utilize various software applications, including Microsoft Office, HRIS, EHR, and other online platforms, often managing multiple logins at once. You possess the ability to obtain an Ohio Driver's License, maintain auto insurance, achieve CPR/First Aid certification, and are prepared to undergo background checks and fingerprinting.
This is a daytime role, Monday through Friday, based in an office setting. There are no weekend or on-call hours.
Willingness to travel to various offices once a week is required.
This position can also be flexible for part-time candidates willing to work a minimum of 20 hours per week.

Under the supervision of the Nursing supervisor, you will support medical personnel with examinations and treatments, execute medical staff orders, schedule appointments, act as a liaison between medical staff and clients, review new client health assessment forms, and communicate recommendations for medical evaluations or further health assessments to the clinical team.
How you'll make a difference in your role:
- Facilitate new referrals by reviewing client health assessment forms to identify any allergies or physical conditions relevant to service management.
- Assist clients in finding suitable health services and resources, including lab work and initial contact with service providers.
- Prepare and maintain client medical files, document health issues, obtain medication consent, and discuss potential side effects of medications with clients and their families.
- Compile monthly nursing reports for the medical section of clinical files and report outcomes.
- Manage medical supplies and submit recommendations for procurement.
- Deliver community treatment services through direct engagement with clients, families, and other essential parties.
- Document billable units using agency scheduling and clinical service software.
- Obtain and facilitate authorization for the release of information for Med/Somatic consumers.
- Review treatment plans and conduct 90-day reviews of Med/Somatic goals with clients.
- Administer injections, perform venipunctures, conduct drug screenings, provide medication education, and oversee logging.
