Job SummaryWe are seeking an experienced Nurse Practitioner to join our team of compassionate healthcare professionals. As a key member of our geriatric care team, you will provide primary, urgent, and acute medical care to seniors with chronic and acute illnesses or injuries.
Responsibilities- Deliver high-quality, patient-centered care to seniors in their homes
- Conduct initial patient screenings and physical examinations, creating appropriate assessment and treatment plans
- Address urgent care needs of patients as needed, collaborating with community teams to complete care assessments
- Order, interpret, and analyze diagnostic tests, determining the need for follow-up appointments and further treatment options
- Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents
- Update patient records and ensure accuracy at each patient appointment
- Deliver palliative care and end-of-life care to patients and families, facilitating family meetings and goal-setting
- Master's degree in Nursing
- Valid nurse practitioner or physician assistant license in the state of operations and current board certification
- 5+ years of progressive care experience
- BLS and ACLS certification, NPI#, and valid driver's license

This is a full-time, permanent position with a regular shift, 8am-5pm, Monday-Friday. Light on-call, no evening or weekend hours unless desired. Nursing experience and a passion for geriatric care are essential for this role.
