Key Responsibilities
* Collaborate with healthcare providers to develop and implement individualized care plans that maximize patient health and wellness.
* Provide direct patient care, including assessment, evaluation, and education, to ensure optimal health outcomes.
* Work closely with clinical staff to ensure seamless care coordination and communication.
Requirements
* 2-5 years of experience in ambulatory care or a related field.
* Basic Life Support (BLS) certification and RN licensure.
* Bachelor's degree in Nursing preferred.
