Job Summary

We are seeking a Registered Nurse to join our team as a Home Health RN. The ideal candidate will have at least one year of experience as a Registered Nurse from an accredited institution and be able to make home visits in potentially unsafe and unsanitary home environments.

Responsibilities
  • Provide and coordinate multidisciplinary home health services for patients in the community
  • Work with the physician and other team members in the implementation of the plan of care
  • Accept case assignment depending on client's needs in specific geographic location(s)
  • Ensure in-person completion of the Oasis that accurately reflects clinical and functional status of the patient
  • Obtain and evaluate HHRG and HIPPS score and work with the Supervisor on matching the scores to frequencies and duration of services on 485 (physician orders)
  • Participate in weekly PPS meetings to report adherence to frequency and duration of services as per physician's order
  • Act as a coordinator of the multidisciplinary team providing care to patients and establishes a care plan with input from appropriate health team members
Requirements
  • At least one year of experience as a Registered Nurse from an accredited institution
  • Prior nursing experience in long-term care or CHHA is preferred but not required
  • Ability to make home visits in potentially unsafe and unsanitary home environments
  • Access to reliable transportation or ability to access public transportation reliably
