**Job Description**

We're seeking Board-Certified/Board Eligible Family or Internal Medicine Physicians to join our team-based primary care practice.

**Key Responsibilities:**

- Provide high-quality, patient-centered care to patients of all ages

- Collaborate with our cross-trained care management and population health teams to provide comprehensive care

- Participate in our innovative clinical model, which emphasizes teamwork, communication, and patient-centered care

- Enjoy a robust clinical and operations framework that supports your ability to take care of patients

- Take advantage of our easy-to-use, optimized EMR system with full telehealth capability

- Enjoy high earnings potential and flexible scheduling

- Participate in our Quarterly Connects with leadership and practice meetings to foster communication and transparency

**Requirements:**

- Board certification/eligibility in Family Medicine or Internal Medicine

- 2+ years of clinical outpatient experience, post-residency preferred

- Current, unrestricted State license or eligibility for licensure

- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)

- Active registration with the Drug Enforcement Agency (DEA)

- Active Medicare/Medicaid enrollment or eligibility to enroll

- Comfortable with Electronic Medical Record (EMR) system
