Key Responsibilities
  • Utilize clinical knowledge and skills to provide care for individuals with physical disabilities and chronic illnesses.
  • Engage in patient and family education, employing suitable resources to create personalized teaching and discharge plans.
  • Deliver hands-on nursing care, applying the nursing process to achieve optimal patient outcomes.
  • Support the care provided by fellow team members.
  • Oversee and guide unlicensed assistive personnel, demonstrating appropriate clinical judgment and effective delegation.
  • Coordinate nursing care in collaboration with the interdisciplinary rehabilitation team to meet overall goals.
  • Exhibit strong oral and written communication skills to build rapport with patients, families, and healthcare team members, ensuring compliance with legal documentation and reimbursement requirements.
  • Maintain a strong commitment to the Acute Care team.
  • Contribute to assessing the physical, psychological, sociocultural, and spiritual needs of patients and their families.
  • Collaborate with rehabilitation team members to enhance the interdisciplinary care plan.
  • Implement the interdisciplinary care plan within the LPN scope of practice, providing necessary nursing care and education.
  • Assist in evaluating nursing care and modifying plans to achieve measurable goals.
  • Participate in interdisciplinary team meetings, contributing to decision-making within your scope of practice.
Qualifications

Essential qualifications required for this position.

Education:

Completion of an accredited licensed practical nurse program.

Experience:

Experience or clinical exposure in Post-Acute Care or Acute Care settings.

Licensure/Certification:

Current licensure in good standing as a Licensed Practical Nurse in Iowa.

Must obtain Basic Life Support (BLS) certification within three months of hire.

Valid driver's license required for work-related travel.

Knowledge, Skills, and Abilities:

Proficient in reading, writing, and speaking fluent English.

Multicultural sensitivity.

Basic computer skills, including Microsoft Office.

Patient-focused with strong critical thinking abilities.

Other:

Familiarity with standard equipment used in the role.

Occasional travel to other facilities may be required, utilizing a UPH vehicle or personal vehicle.
