Key Responsibilities

2. Focus on the pharmaceutical care of patients with acute infections and chronic diseases, working alongside interdisciplinary teams to manage treatment plans.
3. Evaluate and recommend appropriate antibiotic therapies for patients with suspected infections, ensuring optimal medication use.
4. Monitor patient responses to therapies, ensuring safety and effectiveness of medications.
5. Provide comprehensive patient education on medication adherence and disease management, facilitating understanding of treatment plans.
6. Document all patient interactions and care plans in the electronic medical record, maintaining accurate and timely records.
7. Collaborate with healthcare teams to develop and implement patient care strategies, promoting adherence and follow-up care.
8. Assist patients in overcoming financial barriers to therapy by coordinating with patient assistance programs.
9. Participate in educational initiatives for clinical staff, enhancing knowledge and skills related to pharmaceutical care.
10. Mentor pharmacy students and residents, contributing to their professional development.
Qualifications
1. Valid New York State pharmacy license and current registration.
2. Certification in Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
