Job Title: Speech/Language Pathologist

Job Summary:

Cynet Health is seeking a skilled Speech/Language Pathologist to join our team of healthcare professionals. As a Speech/Language Pathologist, you will be responsible for providing high-quality care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities:

  • Provide speech and language therapy services to patients in their homes, including assessment, diagnosis, and treatment of communication and swallowing disorders.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare team members.
  • Conduct home visits and periodic office meetings to assess patient progress and adjust care plans as needed.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses to ensure high-quality care is provided.
  • Travel to home visits and office meetings, requiring reliable transportation and a current driver's license.
  • Handle a caseload of 5-10 patients per day, with a weekly expectation of 30 points.
  • Participate in on-call rotations and be available to discuss weekend work requirements during the interview process.

Requirements:

  • Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience.
  • Must have a valid license in hand; pending licenses are not accepted.
  • Proficiency in HomeCare HomeBase (HCHB) EMR system and must have used it within the last 6 months.
  • CPR and BLS certifications.
