Job Summary: We are seeking a skilled Community Health Worker to join our team at Delta Health Alliance. As a Community Health Worker, you will play a vital role in promoting healthy lifestyles and providing support services to our participants.
Key Responsibilities:
- Conduct individual home visits with participants to provide guidance and support in achieving their health goals.
- Manage a caseload of up to 75 participants, maintaining accurate records and ensuring all administrative tasks are completed in a timely manner.
- Provide evidence-based curriculum and emotional support to participants.
- Maintain confidentiality of all participants and communicate effectively with parents, professionals, and other agencies.
- Plan and implement activities and educational classes that are culturally sensitive and developmentally appropriate.
Requirements:
- Minimum of AA degree.
- Two years' experience in a related field.
- Two years or more as a resident in Lafayette and surrounding counties, with working knowledge of community resources and events.
- Proficient computer skills, including Microsoft Word and Excel, and excellent oral and written communication skills.
- Valid Mississippi Driver's License, proof of insurance, and reliable transportation.
