Renal Dialysis Social Worker - Complex Care Specialist

At LifeBridge Health, we are seeking a skilled Renal Dialysis Social Worker to join our team. As a Complex Care Social Worker, you will play a critical role in facilitating discharge planning for patients with complex needs.

Key Responsibilities:

  • Develop and implement discharge plans for patients with complex needs, ensuring timely and cost-effective care.
  • Collaborate with the interdisciplinary team to identify and address barriers to discharge, including social determinants of health.
  • Provide education and support to patients and families, empowering them to take an active role in their care.
  • Monitor patient progress and adjust plans as needed to ensure optimal outcomes.

Requirements:

  • Master's degree in Social Work or related field.
  • Current licensure as a LCSW-C or LCPC in the State of Maryland.
  • Minimum of 3 years of experience in social work, with a focus on complex care and renal dialysis.
