Job Summary

Key Responsibilities
  • Provide pharmaceutical care to patients in a decentralized setting, working closely with healthcare teams to ensure optimal patient outcomes.
  • Manage and maintain accurate records of patient medication therapy, including dosing and administration.
  • Collaborate with clinical staff to develop and implement individualized patient care plans.
  • Participate in quality improvement initiatives to enhance patient safety and satisfaction.
  • Develop and maintain strong relationships with healthcare providers and staff to ensure seamless communication and coordination of care.
Requirements
  • 2 years of true hospital pharmacy experience in a similar-sized facility (250 census).
  • Experience with Vanco AUC dosing is required.
  • EPIC, Lexicomp, and Baxter IQ EMR experience.
  • Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care.
