Job Title: Physical Therapist Assistant - Home Care

Join our team as a Physical Therapist Assistant (PTA) and provide high-quality care to patients in their home environment. As a PTA, you will work closely with our interdisciplinary team to develop and implement individualized care plans that promote patient independence and dignity.

Responsibilities:
  • Participate in the development and evaluation of patient care plans in partnership with patients, representatives, and caregivers.
  • Provide services ordered by physicians as indicated in patient care plans.
  • Assist treating physical therapists in documenting patient progress and response to treatment.
  • Counsel and educate patients and caregivers to promote patient progress and meet healthcare-related needs.
Requirements:
  • Currently licensed as a Physical Therapist Assistant in the State of Minnesota.
  • Two years of experience as a PTA preferred.
  • Able to travel within geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • CPR certification.
