QUALIFICATIONS AND EXPERIENCE REQUIREMENTS
  • Licensure: LSW through the State of West Virginia Required
  • High school diploma or G.E.D. equivalent
  • Prior experience in social services within a long-term care setting.
YOUR ROLE

As a Community Support Specialist, your responsibilities will include:
  • Establishing and nurturing effective working relationships with other caregivers to ensure their involvement in and support of all programs/services aimed at enhancing residents' psychosocial well-being.
  • Documenting all grievances and complaints, providing necessary reports to the Director of Social Services.
  • Acting as the resident's advocate at all times, collaborating with direct care staff to ensure that residents' needs are consistently met (e.g., appropriate attire and cleanliness). Monitoring any changes in residents' conditions and reporting them promptly to the RN Charge Nurse.
  • Providing insights to the Director of Social Services that could assist in addressing residents' challenges and better fulfilling their needs.
  • Enhancing the quality of services and/or activities, while broadening the range of opportunities and choices available to residents.
  • Collaborating with the activities department to engage family members and guests in activities whenever feasible.
  • Maintaining confidentiality of all resident and family information at all times.
  • Performing general social services duties.
