Job Summary

Key Responsibilities
  • Value-Based Programs (30%):
    • Serve as the lead coordinator for the Maryland Primary Care Program (MDPCP) and other value-based programs, ensuring administrative and case management requirements are met.
    • Coordinate with the MDPCP case management team to deliver and outreach case management services.
    • Work with physician and operations leadership to develop and implement strategies to improve quality metrics and increase incentive earnings for various MDPCP and other value-based programs.
    • Utilize data and resources to develop, implement, and facilitate practice-wide quality-improvement efforts.
  • Patient Care/Clinical (60%):
    • Develop and maintain the Department's Behavioral Health program, including drafting supporting policies, procedures, and guidelines.
    • Assess patients' psychosocial status and communicate with physicians to report patient progress, needs, concerns, and problems.
    • Work closely with the healthcare team to develop psychosocial plans of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
    • Maintain up-to-date knowledge of the field by participating in continuing education activities.
    • Act as a resource person for other healthcare team members, providing in-service education and consultation on behavioral health matters.
    • Maintain accurate and timely records, including clinical and progress notes, according to department policy.
    • Provide positive customer relations in dealing with patients, families, physicians, and other healthcare professionals.
Requirements
  • Education: Master's degree or PhD in Clinical Psychology.
  • Experience: Minimum of two years of clinical experience in an academic setting, with curriculum development preferred.
  • Certification: Licensed Clinical Social Worker (LCSW) certification is required.
