About the Role:

This is an excellent opportunity for a motivated and compassionate individual to make a meaningful difference in the lives of older adults at Hamilton House. As a Human Services Specialist, you will be responsible for delivering high-quality case management services, including counseling, education, and support.

  • You will work closely with clients to assess their needs, develop personalized service plans, and provide ongoing support to help them achieve their goals.
  • Key responsibilities include conducting intake and assessment screenings, assessing mental and physical health needs and supports, and financial eligibility for benefits and entitlements.
  • You will implement evidence-based interventions to promote positive outcomes, including counseling, education, and assistance with benefits and entitlements.
  • Provide regular check-ins and progress updates to clients, as well as maintain accurate and up-to-date records.
  • Collaborate with internal stakeholders to ensure seamless delivery of services and communicate effectively with clients, colleagues, and external partners.
  • Participate in training and professional development opportunities to enhance your skills and knowledge.
Requirements:

To be successful in this role, you will need:

  • A Bachelor's degree in human services or a related field; MSW/LMSW preferred with at least 2 years relevant experience in social services; knowledge of benefits and entitlements a plus.
  • Experience working with older adult populations and families preferred.
  • Bilingual language Spanish fluency preferred.
  • Strong computer skills, including Microsoft Office.
  • Ability to provide accurate and timely documentation required.
  • Time management skills to coordinate multiple responsibilities and services.
  • Ability to work independently and as part of a team, in both a home and office environment.
  • Maintain client and agency confidentiality.
  • Excellent interpersonal, public speaking, and written communication skills.

Salary: $65,000 - $80,000 per year.
