As a House Calls Nurse Practitioner at Abode Care Partners, you will be part of our team that provides personalized medical care to seniors and patients with complex needs in their homes or assisted living facilities.
You will assess patient medical needs, treat chronic illnesses, order diagnostic tests, and collaborate with healthcare teams to ensure comprehensive care.
About the Role
- You will provide evidence-based medical care by planning, directing, and evaluating patient medical needs.
- You will examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
- You will order and interpret laboratory and diagnostic testing as needed.
- You will collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- You will provide referrals to other healthcare practitioners when necessary.
- You will ensure patient medical records are kept up to date.
Requirements
- You must have an active state Nurse Practitioners license.
- You must have current Nurse Practitioner National Board Certification.
- You must have active state DEA licensure.
- You must have a Master's degree in Nursing (MSN).
- You must have five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- You must have strong interpersonal communication skills – verbal and written.
- You must have excellent time management skills with accountability for practitioner's daily workflow.
- You must have proficiency in medical instruments and equipment.
- You must have strong knowledge base of HIPAA and OSHA regulations.
- You must have demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- You must have a licensed driver with valid automobile insurance.
- You must be able to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
