Position Overview

The Physical Therapist will deliver tailored therapy services to patients in their homes, guided by medical directives and individual patient needs. This role includes educating patients and caregivers, as well as coordinating necessary adaptive equipment.

Key Responsibilities

  • Conduct thorough evaluations to determine the extent of disability, establish treatment goals, and develop a comprehensive treatment plan.
  • Implement treatment programs aimed at restoring optimal function, regularly assessing progress and adjusting methods as required.
  • Maintain accurate documentation of evaluations and treatments in accordance with departmental protocols and legal standards.
  • Provide training and guidance to rehabilitation staff, nursing personnel, and students regarding patient care and rehabilitation techniques.
  • Engage in continuous improvement initiatives, focusing on patient satisfaction, program development, and marketing efforts.
  • Demonstrate productivity levels in line with departmental expectations.
  • Enhance professional skills through ongoing education, competency participation, and innovative patient care practices.

Qualifications

  • Bachelor's Degree or equivalent from an accredited physical therapy program (Required)
  • Master's Degree (Preferred)
  • Minimum of 6 months relevant experience (Preferred)
  • State of Michigan Physical Therapist License (Required)
  • Basic Life Support (BLS) certification from AHA or ARC within 90 days of hire (Required)

Physical Requirements

  • Ability to lift and carry weights occasionally up to 50 lbs.
  • Frequent standing and walking, with occasional bending and reaching.
  • Ability to perform various physical tasks as required by patient care.

Work Environment
