Clinical Pharmacist Specialist

This position is a split role, with 50% of service dedicated to ICU patients and 50% to Emergency Medicine patients.
The work schedule is approximately five weeks of days and one week of evenings in a six-week block, with every fourth weekend covered. Evening shift and weekend duties rotate between ICU and Emergency Medicine patients.
Key Responsibilities:- Provide safe, effective, and prompt medication therapy.
- Direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel.
- Collaborate with interdisciplinary teams to deliver optimal medication therapy to patients with diverse disease states.
- Design and implement new pharmacy programs, quality improvement changes, and continuous quality assurance initiatives.
- Develop strategies to optimize medication utilization, including clinical guidelines, disease-specific pathways, formulary management, and medication-use policies.
- Provide departmental and institutional leadership for committees, hospital groups, and external programs.
- Facilitate experiential education and preceptorship, including mentoring learners in implementing improvements in the medication-use system.
- Participate in the management of medical emergencies and provide evidence-based medication education to healthcare professionals.
- Eligibility for licensure to practice pharmacy in Massachusetts.
- Postgraduate residency/fellowship training is preferred.
- Board Certification in the relevant specialty is preferred, required within 18 months of hiring.
- Massachusetts Controlled Substance Registration license, National Provider Index number, and a Drug Enforcement Administration Registration number, when applicable.
- Completion of ASHP-accredited PGY1 and PGY2 Critical Care Pharmacy Residency or Fellowship.
- Able to lift up to 20 lbs.
- Able to stand for extended periods of time.
- Able to sit for extended periods of time.
