Public Health Nurse II

The County of Gaston is seeking a skilled Public Health Nurse II to join our team. The ideal candidate will possess a strong background in nursing and a passion for community health.
Clinical Responsibilities:
The Public Health Nurse II will be responsible for assessing, interviewing, triaging, educating, and implementing care plans for patients. This will include completing vital signs, interviews, and patient/family education, as well as administering immunizations and documenting in the NCIR system. The nurse will also assist with procedures and provide treatments per clinic guidelines, participate in all clinical programs, and review encounter accuracy.
Leadership and Collaboration:
As a Public Health Nurse II, the successful candidate will model positive teamwork and professional communication. They will perform duties of a Charge Nurse as needed, including assessing and directing clinic flow, general problem-solving, delegating duties, and preparing daily schedules. The nurse will also be responsible for assigned follow-up tasks to ensure contract addenda requirements and clinic-specific needs are met.
Referrals and Triage:
The Public Health Nurse II will complete pre-authorization processes, make appointments for needed services, contact patients via phone or mail to advise of recommended plans of care, and document in patient records per policy. They will also follow up to ensure plans of care are completed and contact attempts and closing of patients to follow-up will be completed per policy.
Quality Assurance and Quality Improvement:
The successful candidate will complete monthly program audits of at least 5 records and collaborate with coworkers, medical providers, supervisors, PHS Management, and other departments to ensure best practices within the department. They will also provide input and feedback to the supervisor regarding policies and procedures and clinic operations.
Education and Competence:
The Public Health Nurse II will participate in yearly competence activities for PHS Division staff, participate in in-services, trainings, and meetings, serve as a preceptor to new staff, and complete required competencies per NC Board of Nursing to ensure compliance with nursing license.
Disaster Response and Community Outreach:
The successful candidate will assist the community during emergencies such as natural disasters, pandemics, or bio-terrorism events by delivering services in the field as needed. They will participate in staffing emergency shelters, administering vaccinations to the community, and providing nursing care to ensure community safety. The Public Health Nurse II will also deliver nursing services through community-based clinics, home visits, case management, and community outreach programs, providing education and resources to underserved populations.
Minimum Qualifications:
The ideal candidate will possess a Bachelor's degree in Nursing with a Public Health Nursing rotation or an Associate's degree or diploma in Nursing with one year of professional nursing experience. A valid RN license in the state of North Carolina and a valid BCLS CPR certification are also required. A valid driver's license is necessary for this position.
