Job Title: Physical Therapist - Home Health

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Raleigh, North Carolina.

About the Job

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in the comfort of their own homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients, from infants to geriatric patients, and make a meaningful difference in their lives.

Responsibilities
  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility and range of motion assessments.
  • Implement evidence-based interventions to promote wound healing and prevent further injury.
  • Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
  • Continually assess and revise the care plan, and participate in multidisciplinary care planning as needed.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art electronic medical record system.
Qualifications
  • A current North Carolina Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
