Job Title: Clinical Nurse LPN

Mid-Delta is seeking a skilled Clinical Nurse LPN to join our team. As a Clinical Nurse LPN, you will play a vital role in providing high-quality patient care and supporting our healthcare team.

Job Summary:

The Clinical Nurse LPN will work closely with our healthcare providers to deliver primary healthcare services to patients. This includes preparing patients for examination, collecting labs and administering injections as ordered, making appointments, and navigating patients through the healthcare system.

Responsibilities:
  • Patient Preparation: Prepare patients for examination, including taking vital signs, height, and weight, and ensuring smooth patient flow throughout the clinic.
  • Lab Collection and Administration: Collect labs and administer injections as ordered by the clinician.
  • Appointment Scheduling: Make appointments as ordered by the clinician in a timely manner, notify patients of appointments, and follow up to ensure patients attended and correspondence was received.
  • System Navigation: Be the systems navigator and point of contact for patients and families, with direct access for asking questions and raising concerns.
  • Clinical Information: Assemble information concerning patient's clinical background and referral needs, and provide appropriate clinical information to specialists.
  • Insurance and Review Organizations: Contact review organizations and insurance companies to ensure prior approval requirements are met, and present necessary medical information.
  • Triage and Referrals: Triage patient calls, asking for medical advice from clinicians to answer patient questions, and route med refills to appropriate clinicians.
  • Lab and Diagnostic Imaging: Check the system weekly to ensure that labs/diagnostic imaging or documents have been assigned to a provider.
  • Cultural and Community Resources: Identify and utilize cultural and community resources.
  • Assisting Providers: Assist providers in healthcare activities, performance of diagnostic and/or therapeutic procedures, and the administration of medications as necessary.
  • Exam Room and Supplies: Assist with stocking the examining room and special treatment rooms with needed supplies.
  • Quality Improvement: Participate in care team huddles and/or pre-visit planning to prepare and coordinate patient care activities, and incorporate quality indicators for specific disease states and/or patient variables into patient care plans.
  • Environment and Safety: Ensure an appropriate environment for the administration of healthcare by keeping exam rooms, nurses' station, all equipment, and other work areas in a clean, safe, and orderly fashion.
Qualifications:
  • Certification of practice as a Licensed Practical Nurse or Registered Nurse.
  • Sufficient experience to carry out the duties of this position.
  • Must be qualified in Basic Life Support techniques.
  • Ability to relate with warmth and effectiveness to the patients and to the providers of the community health center.
Evaluation:

The evaluation of work performance will be on-going, and will be carried out by the physician in charge at the clinic assigned, and will include the specific duties and responsibilities of this position description plus employee attitudes and general working behavior. Formal evaluation will normally be performed annually, but may be initiated at other times by the Clinical Nursing Director/Supervisor or the Medical Director.
