Job Description

As a Nurse Practitioner Program Manager at VNS Health, you will oversee the management of service delivery, provide clinical guidance, and support teams to achieve outcomes. You will work under the supervision of the Clinical Director for the Nurse Practitioner Program and be responsible for promoting compliance with quality and cost measures.

About the Role
This position requires a strong background in nursing, with a focus on adult health, family health, or gerontology. The ideal candidate will have a Master's Degree in Nursing, at least two years of experience as a nurse practitioner, and certification by ANCC or another accrediting body. They should also maintain a collaborative practice agreement with a physician and have experience with Hedis and QARR quality measures.

Key Responsibilities
- Manage advanced nursing practice for targeted patient populations
- Evaluate patient responses and revise care plans as necessary
- Identify new/revised clinical protocols and develop training programs

Qualifications
- Strong background in nursing, with a focus on adult health, family health, or gerontology
- Master's Degree in Nursing
- At least two years of experience as a nurse practitioner
- Certification by ANCC or another accrediting body
- Collaborative practice agreement with a physician
