Case Manager

At Fortune Society Inc, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in providing support and guidance to clients with histories of mental illness, substance use, and/or HIV/AIDS.
Key Responsibilities:- Conduct intake interviews and needs assessments to identify client needs and develop individualized treatment plans.
- Provide counseling and supportive services to clients to help them achieve their goals.
- Collaborate with government agencies and community organizations to ensure clients receive necessary entitlements and benefits.
- Act as a liaison between clients and landlords to resolve any issues that may arise.
- Enter client information into our proprietary database and submit reports on client intake and discharge activities.
- High School diploma or equivalent; some college preferred.
- At least two years of experience working with clients with histories of mental illness, substance use, and/or HIV/AIDS.
- Strong follow-up, organizational, and leadership skills.
- Ability to work with disenfranchised populations and encourage them to seek and obtain needed social services.
- Valid driver's license and proven driving ability.
